That dialogue makes no sense with any of the choices. If it's really an Italian dish (authentic or not) then I don't see how it can be found only in Thailand.
 
Italian food that can only be found in Thailand? That would be like Thai food that could only be found in Italy.

It's poor grammar. The problem isn't the "an authentic" part. It's the "It's really" part. I notice with my Spanish students that they frequently use things like "It's really a lovely day" instead of "It's a really lovely day", or "He's really a great guy" instead of "He's a really great guy". Also, they use "really" instead of "actually", which would fit much better in your original.

You can say that as a standalone sentence but it still doesn't work in the context of your original dialogue. It means the same as "It is an authentic Italian dish".

I don't think this is a flight from Istanbul to Paris, but it's possible a flight from Rome to some place... that's why the passenger thought the food on the plane was authentic, because it can only be found in Italy.

It's grammatically correct. Here, "really" is simply an alternative to "very".
 
I think "spicy" should go in the blank. It should be one of the choices. (It's a better choice than "authentic" is.)

That would leave us with It was really spicy Italian dish. This is not a correct sentence.
 
I didn't say I wouldn't change anything else. Perhaps:

It was a really spicy Italian dish. And I like any kind of Italian food.
