---I'm terribly sorry that I have forgotten the reference book you lent me yesterday.
--- _______!
A. Never mind B. Not at all C. You're welcome D. With pleasure

Which of them should I choose?

Thanks!

To be honest, none of the answers seem to be an appropriate response to the given statement.

A more natural response would be something like:

"Don't worry about it."
or
"No problem. Just remember to bring it tomorrow."

It would make more sense to me if we had asked a question first in the scenario. "Oh, never mind, then" would sound more appropriate at that point. While I see that it could work, it sounds like an odd and vaguely rude response to an apology.

I think "Never mind" sounds fine. It would sound odd to me as the response to a question.

I'm really sorry but I've forgotten to bring that ten pounds I owe you.
Never mind. Just bring it tomorrow.

I'm so sorry but I can't make the meeting we arranged for tomorrow morning.
Never mind. We can do it any time this week.

For me, "Never mind" is the equivalent of "It doesn't matter", "Don't worry about it", "That's OK".

Especially with an exclamation point at the end.

Especially with an exclamation point at the end.

It's not a very good question, and there's certainly no need for the exclamation mark if a) is the answer.

Hi trencyfu,
Since you're new I should point out that we generally like you to indicate which you think is right, or which ones you can rule out - ie. some evidence that you've thought about it.
In this case, it hasn't done you any harm, but this won't always be the case.
