"He opened the book at page 10."
"He opened the book on page 10."
"He opened the book to page 10."

How are these different? Are some of them wrong?
 
In the US, you would use only "to."
 
Suppose I accidentally dropped a book and:

"The book fell open at page 10."
"The book fell open on page 10."
"The book fell open to page 10."

How are these different then? Are some of them wrong?
 
As a BE speaker, I'd say at.
