It's painful. It's painful when someone we care about betrays us.
It's painfully. It's painfully when someone we care about betrays us.

Is the second sentence correct?
 
"Painful" is an adjective. It goes after "to be." You only use "painfully" (an adverb) to accompany a verb. Ex. It hit me painfully.
 
You only use "painfully" (an adverb) to [STRIKE]accompany[/STRIKE] modify a verb. [STRIKE]Ex.[/STRIKE] For example, it hit me painfully.
That's not entirely correct. An adverb can also modify an adjective.

That was a painfully slow journey.
