1. Please reinitiate the medical insurance facilities to the following staff members.
2. Please reinstate the medical insurance facilities to the following staff members.

Would please tell me which one is grammatically correct?

The grammar is fine for both. However, I don't know what "reinitiate" means so I wouldn't use that one.

Transitive verb: to initiate something again.

You reinstate insurance that has lapsed, not "reinitiate".

I am quite familiar with the "re" prefix.

Please note that asking about the grammar of a sentence is different from asking if it means what you want it to mean.
