Request to record it the debited amount

Please help me to improve my letter.
Respected sir,
I tried to withdraw an amount of xxxx(amount) from City union bank ATM on 15/4/17.While doing so my account got debited but I have not received the money from the ATM.Therefore I request you to kindly recredit the same amount into my account.I have enclosed a ATM receipt for your perusal.
