The answer is :1-She didn't come to school. She might be ill.

Or

2-She didn't come to school. She might have been ill.

I think the sentence is wrong. He should have said,
1-' She didn't come to school. Perhaps, she was ill.
Or
1-' She isn't 't at school.Perhaps, she is ill.
 
The instructions say to rewrite the sentence. She might be ill is a way to say Perhaps she is ill.
 
Perhaps she is ill = She might be ill

Perhaps she was ill = She might have been ill

Yes, that's absolutely fine. The first sentence is a fact about the past, and the second is a speculation about the present based on the past fact.

Logically, yes, but in this exercise the teacher wants the learner to transform Perhaps she is ill into She might be ill.
