There's maybe 11 women in this city who haven't saluted his old flag.
This is a quote from Two and a half men. I'm getting the impression that it means the same as

There's maybe 11 women in this city who haven't had sex with him.

Am I right? Is this an idiomatic expression? It certainly doesn't seem to be common because I couldn't find it in the dictionaries I own.

No, it's not an idiom.
 
It could have that meaning in the context, but as SoothingDave says, it's not an idiom. UrnamDictionary does give a sexual definition for salute.
