I would like to know the difference between the following two sentences:

(i)With every effort has been made to ensure the accuracy of the Glossary, we would like to apologise should there have been any error or omission.

(ii)With every effort has been made to ensure the accuracy of the Glossary, we would like to apologise should there be any error or omission.

Neither sentence is correct. "With every effort has been made..." There is something wrong here.

Thanks to both bhasisahab and fivejedjon. However, aside from this, what is the difference between the use of "should have been/be in the sentence then?

If I had to choose one of those two, it would be the second. However, more natural would be "we would like to apologise for any error or admission."

The first is apologising for any errors or omissions that were made at the time.
The second is apologising for any errors or omissions that exist now.
Naturally, the two sets of errors and omissions are likely to be the same. It just depends on whether you want to apologise for making them, or for their existence.
