Hello again,

In my opinion, the whole sentence "In 2001 the Centers for Medicare and Medicaid Services (CMS) allowed states to opt out of the requirement for reimbursement that a surgeon or anesthesiologist oversee the provision of anesthesia by certified registered nurse anesthetists" means "according to which/so that/in order that a surgeon or anesthesiologist oversee the provision of anesthesia by certified registered nurse anesthetists".

Am I on the right track?

If "according to which" is the right understanding, then it applies to the requirement.

You chose the wrong phrase. The relevant phrase is:

the requirement for reimbursement that a surgeon or anesthesiologist oversee the provision of anesthesia by certified registered nurse anesthetists

Do you really need to have that explained to you? (As hard as it was to type all that, it would be even harder to explain.)
 
I would be grateful if you would explain to me the fragment you chose.
