Can I use both "in/at" in #3 and #5 and "on/at" in #8?
 
I'd use all the prepositions you've selected (British English).

American English speakers would use 'on' in #8.

If "in university", "in hospital", "in school" are correct in American English, then the book shouldn't be asking to circle the correct option.

Does the author make any attempt (in an introduction or preamble for example) to specify which variety of English her book is concentrating on?

"In school" is possible in American English. We don't say "in university" or "in hospital".

This is the British version of New English File, not the American version.
