Yes, the prescription card, you have to take it to counter and they put the date on it on which you arrive at the hospital along with the stamp of the hospital.

It's an appointment as well as precription card. Could you please tell me the correct way to say these sentences?

I suppose if they stamp and date it, you can say that - ie. that they stamped and dated it.
