Sentence 1 is correct. Imagine it like this:

I need more apples.
How many more apples do you need?
I need three more [apples].

The word "three" goes in the same place as "how many", ie before the word "more".

I can't make that interpretation work for me.

It's not a standard grouping for apples, so it doesn't work. If you said 'I need more dozen eggs', it would be a mess, but I would know what you wanted .
