Hi,

What preposition that collocates with heavy breath?

'Danny was explaining how he got the bruise on his forehead under/in heavy breath. He had been swimming and hit his head on a rock.

"Heavy breath" isn't natural. Are you thinking of while breathing heavily?
 
Your sentence doesn't make much sense in its current form, and I'm not completely sure I know what you mean. Perhaps something like this:

Breathing heavily, Danny explained how he got the bruise on his forehead.
