What is this called? Spot? Freckle?

The was a spot beneath the right part of her lip, making her cuter.
 
It looks more solid (and permanent) than a freckle. I think it's a (facial) mole.

Isn't mole a negative thing? Like a skin abnormality or something like that? Do people find moles gross?

More naturally, The beauty mark below her lower lip made her even cuter in his eyes. If context doesn't make it clear, you should establish that this is a subjective assessment.

I'm not really sure how to phrase a masculine version.
 
In the UK, Cindy Crawford's facial mole was called a "beauty spot". I don't think I've ever heard the same term used for a man's facial mole but, as far as I can tell, it only seems to be women on whom such a mole is considered an enhancement to their looks. On everyone else, it's neutral.
