Why would anyone "send" you homework?
Suppose that I have a friend and he is a native English speaker. He is also a teacher. He always email me some homework to do.
But yesterday, he forgot to email it to me.
Any thoughts?

Bearing in mind that 1 and 5 are identical, they all look OK to me.
 
Your suggestions are all grammatically correct but I would prefer "You forgot to email me my homework yesterday."
