Would it be possible to say:

Do you remember him? A guy from the park, with the twins...

Better:

Do you remember that guy at the park? He had twin boys (or girls) with him.

No, it's "the guy" with "the twins." There is a specific, definite guy you are talking about, so you use the definite article.

In the right context, yes. In the context given, where you expect someone to remember a certain, definite "guy," you need to use the definite article "the."
 
I was thinking like:

How can't you remember him? (He is just) A GUY from the park with twins. (one of many we know)...
 
The "with twins" is pretty specific. Unless there are lots of guys with twins in your neighborhood.

or

Why can't you remember him?

I am not sure which of those you intend to say, but you can't say How can't you remember him? (Okay, you can, but you shouldn't.)

You know a lot of guys with twins? (That doesn't seem likely.)
