The difference in meaning that I see is rather subtle and only stands out in certain cases.

I think your difference in meaning is clear enough. I like it.

In most cases, I don't think it matters from a semantic standpoint which construction is used, so those of you who reject BE accustomed to [base V] can proceed in your unimpeded use of BE accustomed to V-ing instead.

If I accept the way you've explained the difference in meaning (which I do), it definitely does matter to me.

Otherwise, we're left with saying that there are two different ways of saying the same thing. That doesn't seem right. Why on earth would somebody use the to-V construction when they could use the far more common to V-ing construction to say the same thing?
