Lefty asks Pistone:

Lefty: You don't got a family?
Pistone: No, I'm an orphan. I grew up in an orphanage.
Lefty: Ain't the question, even if you had a family.

What does "Ain't the question" mean?
It means he doesn’t care about the orphan part, and is basically saying or implying birth family isn’t the only family out there?

The bold text means "That's not the matter under discussion." Is he referring to something earlier in the dialog?

We need prior context to know what Lefty means. It seems he's using the word 'family' in a special sense.
