"And" in numbers

When is the "and" optional in numbers?

1. "One hundred and six" 106.

2. "One thousand five hundred and six" 1506.

3. 123,456,786

"One hundred and twenty-three million, four hundred and fifty-six thousand, seven hundred and eighty-nine."
 
Out of curiosity, Rachel, why did you write the number out in words then digits in 1 and 2, but digits then words in 3? Of course, it doesn't make any difference to our answer but you should aim for consistency in your written English, regardless of the subject.

"One hundred and twenty-three million, four hundred and fifty-six thousand, seven hundred and eighty-six."

My shots are typical/not unusual in AmE.
