Which are correct:

1) Tom approached us smiling.
2) Tom approached us, smiling.

3) Tom approached us angry.
4) Tom approached us, angry.

5) Tom approached us angry and frustrated.
6) Tom approached us, angry and frustrated.

I don't know why '3' and '4' sound incorrect to me and the others sound more or less fine.

They all seem fine to me. Perhaps what is bothering you is that it's a bit harder to think of a context in which 3 or 4 would be used, whereas the others are commonplace.

I would only use the ones with commas. In fact_ for 3/4 I would say something like:

Tim approached us, shouting and gesturing wildly.
