I myself answered 4 since it didn't feel bad, and the answer key gives 4 too and doesn't explain that.

I wonder why it should be the only correct answer?since 1) seems fitting too, and maybe even 2)

Yes, I would have chosen 4 - though it's not strictly correct.

The problem with 4. (and 1) is that, even if a baby tugging an ear is a sign that the baby is in pain, the baby does not do it as a sign of pain. (The baby does not think, "I'll tug my ear so that they'll know I'm in pain.")
"Children who cannot yet tug may tug on the ear as a sign of pain."
No. They may tug on their ear in an attempt to free themselves of pain, or as a simple reflex, and it may alert the parent that the baby has ear pain, but the child is not doing it as a sign (I believe).

It's still as, though everything Ray says makes sense to me as to why it's a poorly expressed idea.
 
What does "tug on" mean?
