[General] Communication failure or communication failed

There is a communication failed between shipping department with our department. They didn't informed us we have a cargo have to be dispatch.

Is communication failure or communication failed to be used in sentence above?

'There is a' should be followed by a noun.

not a teacher

As "There is a"+ noun, "There is a communication" works for me. Then "failed" is its verb.

My suggestions:
There [STRIKE]is[/STRIKE] was a communication failed between shipping department [STRIKE]with[/STRIKE] and our department. They didn't inform[STRIKE]ed[/STRIKE] us we [STRIKE]have[/STRIKE] had a cargo which [STRIKE]have[/STRIKE] had to be dispatched.

I'm not sure about "shipping department" or "the shipping department" or "a shipping department".

There was a communication failure between the shipping department and our department. They didn't inform us that there was a cargo which had/needed to be despatched.
