B: Why did you curse (at) him?

A: I didn't curse (at) him, I cursed (at) the pain.

I 'curse/cursed' used naturally here? And would you add 'at' all three places in the dialog? Thank you.

Yes and yes. You could also use "swore," with the same use of "at."

"I didn't swear at him, I swore at the pain."

That needs either a full stop (period in AmE) or a semicolon (not a comma) in the middle.

I think there is slight difference with and without using "at". "At" implies "on the spot" whereas without the "it" is not necessarily so.
