I want to say evaluation can be done thirty days after the surgery.

Is my sentence correct? Is it correct to use "has been" at the end of the sentence like I did?

Why are you still trying to make sentences unnecessarily long and complicated? Do you think it somehow sounds more formal/official?

Use the simple past - is done. Or omit it and say "after (the) surgery".

You can use "is done", "has been done" or nothing. I recommend the last option.
