Evaluation medical service can only be performed thirty days after the surgery has been performed.

I want to say evaluation can be done thirty days after the surgery.

Is my sentence correct? Is it correct to use "has been" at the end of the sentence like I did?

Try that.

I've noticed that you frequently give us a better sentence when you're explaining what you're trying to say. There are fewer errors in "I want to say evaluation can be done thirty days after the surgery" than in your longer original sentence. Why are you still trying to make sentences unnecessarily long and complicated? Do you think it somehow sounds more formal/official?

You should think about things in their simplest terms.

Q: When can the medical evaluation be performed?
A: Thirty days after surgery.

The sentence you're trying to write can be made using just the words in the question and answer, and omitting one of them. There are 11 words in total in the question and answer. Your sentence should contain just ten of those words.

Use the simple past - is done. Or omit it and say "after (the) surgery".

You can use "is done", "has been done" or nothing. I recommend the last option.
