I would like to ask about a word, failure.
Which is correct?

A system failure has occurred. (countable)
System failure has occurred. (uncountable)

I would use the first, though you could make a context where the second would work.
 
I agree with 5jj, though "There has been a [complete] system failure" would be more natural.
