The felt table top was found to be damaged. The felt was crushed, with screws head impressed on the top. It was due to improperly stacked on top of one another.

Is the sentence above grammatically correct?

One screw head? "a screw head"; Two or more ? "screw heads". "...due to being improperly stacked..."
 
It was due to the tables improperly stacked on top of one another.

Out of curiosity, can a soft material like felt be "crushed"?

For me, the correct way to say this is a combination of the two suggestions above:

... due to the tables being improperly stacked ...

When you say "improperly", do you mean that they should have been stacked in a different way or that they shouldn't have been stacked at all?

They should have been kept a packing paper in between 2 tables. They stacked directly on top of another one; otherwise, it would have not been damaged like this.

Please advise should I write the following?

The felt table top was found to be damaged. The felt was crushed, with screw heads impressed on the top due to the tables being improperly stacked on top of one another.

I would say the felt tabletop was damaged/ruined beyond repair.

I would say the felt tabletop was damaged/ruined beyond repair.
You can say ruined or damaged beyond repair. "Ruined beyond repair" is tautalogous.
 
Crushed might confuse people with crushed velvet
