Could someone please explain the difference between Have you ever ....? and Do you ever ....?
Thank you.

Do you ever eat sushi?
Have you ever eaten sushi?

Why don't you try to explain the difference to us? This might help you think about it. Also, it will help us know what you already know.

Tip: Try thinking about how you would answer the questions if somebody asked them to you.

Do you ever eat sushi? means these days / something you may do regually (Answer: Yes, I do.)
Have you ever eaten sushi? means in the past up until now / something you have actually done (Yes, I have.)

They appear to be very close in meaning but "Do" is something you have done or may do and "Have" is only something you have actually done.

You're on the right track. I don't think they're all that similar in meaning though. The first refers to the present and near past, while the second applies to your entire life. You could answer the first with No, never. Not since I got food poisoning from it fifteen years ago! In that situation your answer to the second might be Yes, but not anymore. I got food poisoning from it fifteen years ago and I haven't touched it since.
