He lost his faith OR He has lost his fate

If I want to say that a man has lost his fate and he hasn`t reclaimed it yet which of the above in the title should I use?

Does He lost his fate say that he lost it in the past but now he`s got it back?

Does He has lost his fate imply that he hasn`t found it to this very day?

First, you have repeatedly used "fate" when it appears that "faith" was intended. One of your sentences uses the past tenses and the other uses the present perfect tense. They are both correct. The uses will depend on context. Neither of the sentences means that he's got it back.
