Hello.
What tense would you choose in this grammar test, Past Perfect Simple or Continuous?

At that time, I _________________________ (not hear) from her for more than three months.

It looks like 'hear' in this case is an active verb, so it can be used in the continuous form.

Only past perfect simple is possible here.

I don't understand your reasoning about 'hear' being an active verb. There's no need for a continuous tense in this sentence.
