1) Would the sentence be wrong if I add the to? "meditation helps to improve."

[...] Meditation also can improve focus, concentration, creativity, and learning and reasoning skills.

2) Could I say: "Meditation can also improve..."?

ad 1) It would be all right. The meaning would not change a bit. You can find this sort of information in any good dictionary or grammar book. For example here help - Definition and pronunciation | Oxford Advanced Learners Dictionary at OxfordLearnersDictionaries.com (see right above the meaning #2 (in a yellow bracket)).

ad 2) You could, although the meaning would be different. It would mean that mediations do not necessarily help, but it is possible that they do.
