I tried filing a claim changing the patient's name.

Is my sentence correct?
 
It's not clear what you're trying to say. You don't file an insurance claim to change someone's name. Also, I'm pretty sure that the only person who can change the name on an insurance policy is the policy holder.

I mean the patient's name was incorrect. I tried to refile after changing the patient's name.
 
Do you mean that the patient's name was incorrectly spelled on the original claim or that you had accidentally given a totally different patient's name?

No. Either you filed a claim or you didn't. Say:

In the original claim the patient's name was misspelled. I filed an amended claim with the patient's name spelled correctly.

If the original claim had the wrong name entirely then say that.

OK, now we're getting somewhere. I suspect that one of two things happened:

1. You submitted a revised claim, this time with the patient's name spelled correctly and you enclosed a covering letter/email explaining the reason for the revised claim.
2. You simply sent a letter/email to the insurance company to tell them that the name was spelled incorrectly on the original claim and asking them to change it to the correct spelling.

Which did you do?
