Hello!

If I am making a statement of habit, saying that "X" (the name of the mall) where I generally go shopping, what is the correct form?

I usually go shopping to/at (the) "X".

I would use 'at'.

It depends. You don't need an article with the name of a store.
"I go shopping at K-Mart."
"I go shopping at the mall/supermarket."

'I usually go shopping at the K-Mart supermarket.'
Is 'the' needed here?

Not a teacher.

Yes, but no one would say that - assuming that K-Mart is a locally known store.
