I usually have confusion on how to use past tense of the verb to be, for instance " I was recommended to make a follow up" or I has been recommended to make a follow up. which one is correct.

"I has been" is incorrect in English. As jutfrank has pointed out above, "I have been" is correct.

I don't know what you mean by "a follow up". Can you give us the longer version of that sentence, or describe the context (situation) in which you intend to use it?

It is possible to use "I was recommended" in some contexts; e.g.,

I was recommended by the head of department for this position.
