I can't say which book this exercise is as the copy I have does't say that. I need to use the conditional sentences. Is sentence #1 correct if I am talking about the present? If I had to talk about the past should it be "Had I been your teacher, I would have insisted on you do more careful work."
1. Were I your teacher, I would insist you do more careful work.

If I had to talk about the past should it be "Had I been your teacher, I would have insisted on you do more careful work"?
Your first sentence is correct when talking about the present.
With my corrections, the other sentence is correct.

Note that whilst I won't say that "do more careful work" is ungrammatical, it would be more common to hear "[that] you take more care with your work".
 
I'm puzzled. Why do you need to use conditional sentences?
