[Grammar] Inverted Subjunctive mood

I'm having trouble deciding between the following two sentences about which is right:

1. Had you not warned her beforehand, she would have made a mistake.

2. Hadn't you warned her beforehand, she would have made a mistake.

#1 is correct, you cannot use the contracted form "hadn't" like that.

Just got another question in my exercise book.

Not until then _____ the emergency of the situation.
A. did he realize B. had he realized

I chose A cuz I think "to realize the emergency" and "then" in the sentence should be simultaneous, so there shouldn't be a time lag so as to use the past participle form of "realize" but somehow the key is B. I'm not sure if the key should be right there and after I searched in some other books and dictionary I found these two sentences:

1. I didn't reach the station until the train had left.
2. Only when I left my parents for Italy did I realize how much I loved them.

So they kind of support my own thought. Any idea please?

For B to be correct it would have to be "realized". It is true that we would usually use the perfect tense there.

Sorry, was my bad to mistyped the word, the B option is actually "had he realized".

Do you mean that even though the two events in the main clause and adverbial clause are simultaneous, you still would use a past perfective instead of simple past right?

With "Not until then..." we are talking about an unspecified period of time before a certain moment: "then".
