Speaking to the mail clerk about filling out an express shipment form. I ask:

"Isn't there a part of this form for you to fill out as well?"

VS

"Isn't there a part of this form that you to fill out as well?"

Are both okay?

"Isn't there a part of this form for you to fill out as well?" This is okay.

VS

"Isn't there a part of this form that you to fill out as well?" Omit 'to' and this is okay.

Are both okay?
