Which one? I'm stuck between "lived" and "have been living".

1, 2 and 5 are all possible.

'Lived' denotes the speaker does not live here any more.
'Have been living' denotes the speaker is still living here.
Correct?

Yes, and of course "shall live" is a future intention. All are possible, so without context it's impossible to say which is correct.
