Looking back on it, I still can’t figure out what went wrong.

I would take it to mean "Now that I am looking back on it". However, it could mean "Whenever I look back on it". I might have looked back on it previously, be looking back on it again now and I might even look back on it again in the future. Of course, if I look back on it in the future, I might suddenly figure out what went wrong!
