I'm trying to write a dialog:

Maybe it's time that I... that I make some changes in my life.

Does this line of dialog sound perfectly natural with 'that' in it or would native speakers almost always leave it out?

It's equally fine with or without. You could also consider using it once, but not twice. You could omit it during the initial thought, or once the character finishes their thought process.

I would use 'made' rather than 'make' in that sentence.
