There was a claim that denied because the patient name read(second form) as "Morris John" where as it should have read as "John Morris".

Is it correct to say that "someone's name should read as"?

Surely, it is the written form of the name that matters in a insurance claim, not how it is read.

It is "read" as it is "written." It's an expression.

The name was written wrong. It should have been given name and then surname. It was backwards, hence the claim was rejected.
