The answer is option (A) , but why isn’t it option (D) ?

Sally’s family only care about themselves._________, they always want her to do more for them and for the house. Home is not sweet for her.

(A) Also
(B) Finally
(C) However
(D) For example

D is a better choice than A.

(I would say, "Sally's family only cares about themselves.")
 
What is the source of the question? Always give us the source of material you quote.
 
I can't speak for AmE but "Why isn't it ...?" is absolutely fine in BrE.

For (D) to work here, you'd have to believe that Sally's family wanting her to do more for them is one example of their only caring about themselves.

For A to work you have to believe there is a reason to use "Also" there.

Yes—that reason being that the second sentence expresses additional, not exemplifying, information. I think that's how most would read it.
 
@EthanChao, you still haven't provided the source and author of the text you quoted. This is a legal requirement on the forum whenever you quote someone else's words. It's a copyright issue.
