should italics be repeated

As I understand it the name of a place should be in italics i.e

we used to go every Saturday night to a dance hall called The Hammersmith Palais.

Should that same place continued to be in Italics after that or is it only in the first instance ?

I would suggest your initial understanding is wrong. There is no reason to italicize it the first time. You certainly would want to do it more than once.

How about Titles ? i.e.

My favourite book is War and Peace.
