In this example the phrase specifically means that you have scored the maximum marks out of a possible ten. For example, there were ten questions and you answered ten correctly.
But idiomatically, "ten out of ten" can have a general meaning of "excellent" or "as good as could be hoped for or expected", even if there is no real points system.
For example: "At your party the music and food was great, ten out of ten!", "I don't like your suit but the shoes are definitely ten out of ten!". The phrase "a perfect ten" has a similar meaning.
