2. Not a single day had passed without Anna having a bad conscience about her parents.
3. Not a day had gone by without John having pain since cancer had spread in his body.

Do you mean 'Anna feeling guilty conscience about her parents'?

That's OK, but it's a bit awkward. Try: "Not a single day passed that Peter didn't think about his former wife."

2. Not a single day had passed without Anna having a bad conscience about her parents.

Try: "Not a single day passed without Anna feeling guilty about her parents."

3. Not a day had gone by without John having pain since cancer had spread in his body.

That's OK, but try: "Not a day went by without John having pain since cancer had spread throughout his body."
