Would you please tell me which one is grammatically correct?

I think it should have said something as follows.
The uniforms have been worn out too much.

We don't normally associate wear and tear with damage
The uniforms have been worn out too much.
That doesn't work.

If you say that the uniforms are completely worn out, the assumption will be that they are unwearable. There is no need to state that.
 
If something is worn out it's worn out. The "too much" is built in to the definition.
