"What is happened"

Why is it incorrect even "happened" refers to "What"
 
Because 'happen' is an intransitive verb, the passive voice is not possible.

Not a teacher.

Hello, Checkmate:

If you are walking down the street and come across (encounter) an automobile accident, you could ask someone:

What happened?
What has happened?
What's happened? [The 's = has]
What is happening? [especially if there still seems to be a lot of confusion in the area]
What's happening? [the 's = is]
