RACHEL: Okay, okay, okay, I got one! (SHE SITS UP AND THE CUSHION SHE WAS LEANING AGAINST FALLS OFF THE BALCONY) Anyway- The valentine Tommy Rollerson left in your locker was really from me.

MONICA: Excuse me?!

RACHEL: Hello? Like he was really gonna send you one? (TO PHOEBE) She was a big girl.

Friends Season1 Episode4

is it mean that "are you sure" or "did you believe"

[It was] Like he was really gonna send you one? (TO PHOEBE) She was a big girl.

"Like" is shortened in speech from "It was like" or "It was as if" or "do you think / suppose" etc...showing doubt that something would occur.

Does it mean, "are you sure" or "did you believe"?

No, neither.

Rachel is using sarcasm to point out the fact that fat girls (ie Monica when she was younger) are unlikely to get Valentines cards from (presumably) attractive boys/men like Tommy Rollerson.

In this context, "Like" should be understood to mean "As if".
