1. Whether might he look like me or not?

2. Whether he might look like me or not?

I want to ask in question form. Which is correct (1, 2) form? -Not sure but 1st
 
Neither is a complete sentence. That said, #1 does not work for me.
 
Neither is a correct sentence. Neither is a question nor an assertion.
You appear to putting forward a proposition "He might look like me" or "He looks like me". And you want to make it a question. The simplest way is "Does he look like me?"
 
In certain contexts, you might be able to make a case for "Might he look like me?"
